Appointments

All Patients

  1. Please arrive 10-15 minutes prior to your scheduled appointment so that we can update or verify your current contact information and insurance, and obtain any pertinent clinical information.
  2. Please bring your most current medication list, blood pressure log, and/or blood glucose readings.
  3. Inform the clinical staff of any new pertinent medical information including new diagnoses, allergies, and recent tests from other providers.
  4. If you were hospitalized or seen by another health care provider, please have your records sent to our office or bring them with you to your appointment for us to review.
  5. Please bring your most up-to-date insurance card.

If you cannot make your scheduled appointment we ask that you let us know 24 hours in advance.

New Patients

Please bring or have sent to our clinic all medical records, including list of medication, allergies, recent blood work, and all diagnostic tests/exam results.

Additionally, please download and complete the patient history forms prior to arriving to your appointment.
